We currently manage several Extra Care schemes nationwide in partnership with local authorities and housing associations. Extra Care provides housing and care enabling frail older people to live more independently in their own homes, while knowing that help and support is on hand, 24 hours a day.
These schemes provide the service user with choice and independence whilst preventing admission to residential, nursing or hospital care.
Key features of the service include:
• 24 hour on site management - scheme manager and a team of trained care staff. • Assistance with personal, social and domestic tasks associated with ordinary living. Overseeing the general welfare of tenants and responding flexibly to day-to-day changes. • Rehabilitation programmes - actively enable and encourage individuals to take responsibility for their own lives and avoid dependency. • Encouraging the service user to remain active and in contact with friends, relatives and the local community. • Assisting with and minimising the effects of disability, mobility, illness/infection and operations. • Respond to care need crisis. • Close liaison between all agencies involved in the provision of care, including social services and district nurses.
Care UK are currently working together with Hanover Housing Association as ‘solutions providers’ in a unique venture, which brings together extensive experience in the delivery of high quality extra care housing and homecare services.
Both organisations have now combined their skills and expertise on three extra care projects. The relationship, although not exclusive, is sufficiently flexible to allow local authorities’ commissioning teams and health and social services to benefit from bespoke solutions that are tailored to meet local needs.
