Home from hospital scheme

We operate a number of home from hospital schemes nationwide, providing a time limited service for patients returning home from hospital after illness, surgery or accident. The emphasis is strongly placed on people achieving full rehabilitation and regaining independence whilst also enabling quicker discharge from hospital.

Many service users are fit to return home, but need assistance in the early days of discharge to regain their confidence and strength. The service provides personal care and/or any other practical assistance for a two to six week period. At the end of this period, the service user is usually able to remain at home without any further help or assistance.
