Outcome based dual assist service

Care UK is working in partnership with Suffolk Social Care to provide the first ever outcome based homecare service - focusing on what is achieved by the service user whilst not being time or task related. A key objective of the service is to enable individuals to manage their own service and how it will be delivered by making their own choices. Other specific objectives include promoting independence and encouraging older people to have an active presence in the community.

Service users receive an initial assessment to determine the level of service required for their needs – this will be between 5 and 30 hours per week. We also work with the service users to agree a set of outcomes taking into consideration:

• Health & Safety
• Autonomy & freedom of choice
• Ability to manage personal & other daily routines
• Involvement in family, other organisations and the wider community.

The service is provided by teams of two care workers, using vehicles provided by Care UK. A care co-ordinator works closely with the care workers and service users to prepare weekly care plans to achieve the agreed outcomes which might include personal care, meal preparation, shopping, social inclusion and community integration.
